• Valid TLC/ DMV Class E License
• No At-Fault Accidents
HOW TO APPLY:

Minimum Application Items:
• A front and back picture of your TLC Driver’s License
• A front and back picture of your Class E DMV License
• Proof of Address less than 30-days old (can be utility bill or credit card statement with matching address)
• Defensive Driver Completion Certificate, and any police reports for any accidents within the last 3 years.
• All Police Reports for any car accident within the past 5 years
• If you have had your DMV License in New York State for less than 5 years, photocopies of previous driver’s licenses.
